1. Find this year’s SBC for every plan you’re considering Summary of Benefits and Coverage — not just the marketing comparison chart. 2. Write down three numbers per plan (individual in-network) Your premium share (per paycheck × pay periods, or monthly × 12); annual deductible; out-of-pocket maximum. 3. Note whether the plan is HSA-eligible If yes, check employer HSA contribution (if any) in the benefits guide — separate from premium. 4. Skim pharmacy If anyone in the household is on a regular or specialty med, check the formulary tier before you enroll. 5. Run the liability math Use the healthcare liability calculator: premium + OOPM = worst-case total, as a share of salary. 6. Ask these before you click enroll Is the individual OOPM embedded in the family OOPM? Do copays count toward deductible, OOPM, or both? What’s in-network for our doctors and preferred hospital? Any prior auth on care or Rx we already know we need? If HSA: what’s the employer seed, if any? 7. Save PDFs Download the SBC (and formulary snapshot if you checked one) before the portal closes. 8. Enroll — then confirm Submit elections. Check the confirmation screen or email so the plan you meant is the plan you got.
